50yr woman with Iron deficiency anemia with hypoproliferative bone marrow

Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.

Here is a case i have seen




Patient was apparently alright 5 days back.Then she had fall from bike (met with accident).she had injury to her teeth.
No h/o loss of consciousness, vomitings,headache , blurring of vision, bleeding from ears.No h/o head trauma.
She went to dental OPD for fixing of teeth.
Her routine investigations revealed anemia , for which she was referred to medicine department.  No h/o fever , burning micturition.No h/o bleeding manifestations.

Not a k/c/o Dm2/HTN/asthama/TB/Epilepsy/Thyroid abnormalities.
No h/o blood transfusion in the past
No previous surgeries done.
No significant family history.
She attained menarche at 12yrs of age          and has regular cycles for 2 days every month .No clots and dysmenorrhoea.
Personal history:Mixed diet ,normal appetite, adequate sleep,normal bowel and bladder movements.
Vitals:Temp-Afebrile
Bp: 110/70 mm hg
PR: 68 bpm
General examination:patient is conscious , coherent and cooperative.Moderately built and moderately nourished.
Pallor+, koilonychia+
No icterus , cyanosis, clubbing,generalised lymphadenopathy or pedal edema.
Cvs : s1s2 heard, no murmurs
Rs: B/l air entry + ,no added sounds
CNS: No abnormality detected.
Treatment:
Inj IRON SUCROSE  1 amp in 100ml NS iv OD
Inj. PAN40mg IV/OD
INJ NERVIGEN 1amp in 100 ml NS iv OD
Tab.PCM 650 MG OD
